Chicago Christian Academy currently accepts the Child Care Assistance Program offered through Illinois Action for Children.

 

The Illinois Child Care Assistance Program (CCAP) makes it possible for your child to enjoy the benefits of quality child care.

 

For your convenience, child care forms have been provided below.

Please read all instructions carefully.  To help serve you better, please make sure forms are filled out completely and legibly.  Attach all necessary documentation (i.e. check stubs, school schedule). Keep a copy of all forms for your records.
